RESUMEN
Varón de 10 años de edad, previamente sano, que en forma brusca comenzó con movimientos paroxísticos e involuntarios en lengua y hemicuerpo derecho. Al momento de su ingreso en el servicio de Neuropediatría presenta  hemicorea derecha con Babinski y hiperreflexia osteotendinosa ipsilaterales.
A nivel del globo pálido izquierdo una lesión hiperintensa en T2 y flair fue observada en la RM cerebral, constatando además señal brillante segmentaria visible en T2 y STIR en proyección T6 del cordón medular.
Durante el seguimiento a lo largo de tres años, dos nuevas recaidas fueron documentadas.
Los hallazgos clínicos e imagenológicos resultan compatibles con esclerosis múltiple infantil de inico inusual.

SUMMARY
A previously healthy 10-year old boy presents with sudden onset of involuntary and paroxysmal movements in the right side of his body and tongue-thrusting. On neurologic examination right-sided choreic movements with deep tendon reflex  symetrically brisk were noted.
Brain MRI showed, on T2 and Flair, a focal bright lesion involving the left globus pallidus; more over at the T6 level of the spinal cord a segmentary hyperintense lesion was seen on T2 and STIR.
During follow-up, along the last three years, two new relapses ocurred.
These clinical and imaging findings were compatible with an unusual onset of  children multiple sclerosis.
