RESUMEN
Hombre de 39 años con dificultad progresiva en la marcha y desorientación temporoespacial de un mes de evolución; pocos días antes de la internación agrega trastornos visuales con incontinencia esfinteriana.
Al ingreso se diagnostica retinitis bilateral y el test rápido para HIV resulta positivo. La RM cerebral evidencia en flair y T2 hiperintensidad de la sustancia blanca bihemisférica a predominio frontal con compromiso calloso.
Postcontraste hay áreas de refuerzo en rodilla callosa e insular profundo bilateral. Fallece al quinto día de internación diagnosticándose por necropsia: leucoencefalopatía multifocal progresiva asociando coroideoretinitis y encefalitis aguda por toxoplasma gondii.

SUMMARY
A 39 year-old-man who presented during the last month progressive gait disturbance and desorientation; morover sphincter disfunction and visual impairment appeared few days before the patient was hospitalized. On admission HIV-seropositivity and ophtalmological diagnosis of bilateral retinitis were the most important findings.
FLAIR and SET2 wheighted-images revealed bilateral high signal in the  frontal white  matter  with  callosal extension; abnormal focal abnormal enhancement was seen in the genu of corpus callosum and in both insular posterior areas.
He died five days after admisission and histopathological examination showed tipical findings of progressive multifocal  leukoencephalopathy in association with toxoplasma gondii choroidretinitis and encephalitis.
